Help with prescriptions

Social Security Extra Help: help paying for your medications

Extra Help is a federal program, run by Social Security, that helps people with limited income and resources pay Part D costs: the premium, the deductible and prescription copays.

Who qualifies? (2026 limits)

Yearly income

Individual
$23,940
Couple
$32,460

Resources (savings and assets)

Individual
$18,090
Couple
$36,100

Your home and car don’t count as resources, and some income isn’t counted either. If you’re a little over, it’s worth applying.

How much do you save?

  • $0 premium in many basic Part D plans.
  • $0 deductible.
  • 2026: no more than $5.10 per generic drug and $12.65 per brand-name drug.
  • 2027: no more than $5.80 per generic and $14.40 per brand-name drug.

You qualify automatically if…

  • You have full Medicaid.
  • You get a Medicare Savings Program (QMB, SLMB or QI).
  • You get SSI (Supplemental Security Income).

In those cases you don’t need to apply: Medicare enrolls you and sends you a letter.

How to apply

  • Online on the Social Security website.
  • By phone at 1-800-772-1213 (TTY 1-800-325-0778), Monday to Friday, 8 a.m. to 7 p.m.

Benefits of having both programs

1

Lower out-of-pocket costs

Depending on your Medicaid level, the state may pay your Part B premium and, in many cases, your Medicare deductibles, coinsurance and copays.

2

Automatic help with prescriptions

If you have Medicaid, you automatically qualify for Extra Help. In 2026 you pay no more than $12.65 per prescription.

3

D-SNP plans made for you

Dual Eligible Special Needs Plans (D-SNPs) coordinate your Medicare and Medicaid benefits. Many include extras like dental, vision, hearing, transportation or an allowance for over-the-counter items. Benefits vary by plan.

4

Long-term care

Medicaid may cover services Medicare doesn't, such as care at home or in a nursing home, if you meet the requirements.

5

More chances to change plans

People with Medicare and Medicaid have special periods during the year to change coverage, not just during annual enrollment.

6

One team to guide you

We help you make sure your doctors and medications are covered and explain how your two programs work together.
